Study finds HIV-infected men at risk for spreading HIV despite taking HAART

Researchers from Boston University School of Medicine (BUSM) and Fenway Health have found that highly active antiretroviral therapy (HAART) does not completely suppress HIV in the semen of sexually active HIV-infected men who have sex with men (MSM). The findings, which currently appear on-line in AIDS, could indicate a potential transmission risk in MSM, who are highly susceptible to HIV infection.

Approximately 33.3 million people worldwide are living with HIV/AIDS, and 1.8 million deaths and 2.6 million new infections occur annually. Unprotected intercourse is the most common route through which HIV-1 is transmitted, and semen of HIV-infected men is an important source of HIV. Whereas the HIV/AIDS epidemic in Sub-Saharan Africa is generalized with approximately equal percentages of infections occurring in men and women, the epidemic in the United States and many other developed countries is concentrated in men who have sex with men. HAART, which is the primary treatment for HIV/AIDS, consists of a combination of potent anti-HIV drugs, which generally suppresses HIV levels in blood and semen, and HIV transmission to sex partners. However, sexually active MSM have a high prevalence of sexually transmitted infections which are a risk factor for HIV transmission.

To determine what percentage of sexually active HIV-infected MSM on HAART have HIV in their semen, the research team, headed by Deborah Anderson, MD, professor of obstetrics and gynecology at BUSM, recruited 101 men from Fenway health, Boston. They measured levels of HIV in blood and semen, and examined the relationship between HIV in semen and other clinical, behavioral and biological variables.

Eighteen percent of the men had HIV in blood despite being on HAART, and half of these men also had HIV in semen. "It has been shown before that HIV levels in peripheral blood are an important predictor of seminal HIV," explained lead author Joseph Politch, PhD, research associate professor in obstetrics and gynecology at BUSM.

Of the 83 men with undetectable HIV in blood plasma, 25 percent had HIV in their semen. Detection of HIV in semen was strongly associated with high-risk (unprotected) sex behavior, sexually transmitted infections and genital inflammation.

"Our study provides evidence that genital infections and inflammation are common in HIV-infected MSM that engage in unprotected intercourse, and that these factors can promote HIV in the genital tract of men on suppressive HAART therapy even when HIV is not detectable in blood," said Politch.

Because MSM are more vulnerable to HIV infection than heterosexual men, this information has potential clinical significance for the HIV epidemic in MSM. "Men who have sex with men who are at risk for transmitting HIV may believe that they have a low risk based on incorrect assumptions that HAART eliminates HIV from semen. Until more information on transmission risk in MSM is available, it would be prudent for sexually active HIV-infected MSM to use condoms and other risk-reduction strategies throughout all stages of HIV disease, regardless of HIV treatment status, and to promote the aggressive diagnosis and treatment of STIs," Politch said. Kenneth Mayer, MD, Director of Research at Fenway Health and a co-investigator on the study added: "We would like to extend this research to study the effects of newer HAART regimens, since newer agents may have different effects on HIV suppression in different compartments in the body, which may have implications for HIV prevention."

Weight-loss surgery cut blood sugar more than drugs

CHICAGO (Reuters) - Weight-loss surgery did a better job of controlling type 2 diabetes in overweight and moderately obese patients than the most advanced medical treatment for the disease, researchers said on Monday.

The study, conducted at the Cleveland Clinic and presented at the annual scientific sessions of the American College of Cardiology in Chicago, showed that patients who underwent surgery were more than three times more likely to gain control over their diabetes after one year than the group that was treated with drugs.

Uncontrolled diabetes is a major risk factor for heart problems, including heart attack.

"Within days and hours of surgery -- before there's any measurable weight loss -- we saw dramatic changes. A majority of(surgery) patients left the hospital with normal blood sugars. However, this was not as effective for people who had diabetes for many years," said Dr. Philip Schauer, director of Bariatric and Metabolic Institute at the Cleveland Clinic, who led the clinical trial.

Shauer called the findings, which were published in the New England Journal of Medicine, "a potential paradigm change" for how some patients should be treated for diabetes.

In the study, dubbed STAMPEDE, researchers randomly assigned 150 patients -- three-quarters of them female -- with a body mass index between 27 and 43 into one of three groups.

There were two surgery groups -- laparoscopic gastric bypass, a surgery that reroutes the digestive system and allows food to bypass part of the small intestine, and sleeve gastrectomy, a procedure that reduces the stomach to about one- quarter of its original size.

The two surgery groups were compared with a third group that got the most advanced non-insulin treatment for diabetes, such as liraglutide, marketed by Novo Nordisk under the brand name Victoza.

The research was primarily funded by Johnson & Johnson's Ethicon Endo-Surgery Inc, a maker of surgical instruments used for bariatric surgery.

REACHING GOAL

The main goal of the study was to reduce blood-sugar levels as measured by a test called HbA1c, a standard tool used to determine blood-sugar control in patients known to have diabetes. The America Diabetes Association recommends an HbA1c goal of less than 7 percent.

Patients in all groups had an average HbA1c level of 9 percent. The study measured those patients who achieved levels 6 percent or lower after one year.

Researchers reported that 42 percent of patients who underwent gastric bypass surgery achieved that goal, compared with 37 percent of patients who got the sleeve gastrectomy. Just 12 percent of the patients in the drug group achieved that goal.

"This study shows, in this group of patients who have poorly controlled diabetes, surgery is more effective than medication alone," Schauer said in an interview. "More doctors who treat diabetes are going to think about surgery for these patients.

"The implication is that insurance companies might need to reconsider coverage. Right now, there's a brick wall at 35," he said, referring to the BMI threshold where insurers will cover surgery.

A BMI is a number that is calculated from a person's weight and height and provides a reliable indicator of body fat for most people.

Not surprisingly, weight loss was five times greater among those who had surgery than those who did not.

While there was only a small improvement in blood pressure and cholesterol, those in the surgery group were able to slowly get off their blood pressure and cholesterol-fighting medications.

About 80 percent of the 23 million Americans living with type 2 diabetes are overweight or obese.

Dr. Steven Nissen, head of Cardiology at the Cleveland Clinic and another investigator on the STAMPEDE study, said half of the patients in his hospital's critical care unit are diabetic, which is typical of most hospitals in the United States.

"One percent of people in the weight range that would qualify for bariatric surgery are getting it. That could go up to 2 to 3 percent" on the basis of this study, Nissen said.

"If you do a $20,000 operation, what are the cost savings over time?" He said the Cleveland Clinic was following those patients from the study for five years to complete an economic study.

"I think more patients will be referred (for bariatric surgery) for consideration by their (insurance) carriers and more insurers will pay for bariatric surgery. They've thought of it as a cosmetic procedure, without considering the profound medical benefits" of the procedure.

He said it was frustrating to see those patients go down hill with vision loss, need for amputations, kidney dialysis and that he has gradually come to the conclusion that surgical procedures are warranted in some cases.

"Obesity has taken the country by storm and we have to stop it," he said.

Liverpool manager Kenny Dalglish hopes local rivals Everton can join them for an all-Merseyside FA Cup semi-final at Wembley next month.

League Cup winners Liverpool booked their second visit of the season to Wembley with a 2-1 quarter-final win over Stoke at Anfield on Sunday.

Soon afterwards the semi-final draw was made and it meant Liverpool would be facing Everton provided the team from the other side of Stanley Park beat Sunderland in a last eight replay next week.

The city rivals have not met at Wembley since Liverpool beat Everton 3-2 to win the 1989 FA Cup final played out against the backdrop of the tragedy that was the Hillsborough disaster.

This year's Wembley semi-finals are scheduled to be played on the weekend of April 14 and 15, the latter date being exactly 23 years since the death of 96 Liverpool fans at the Sheffield ground.

Liverpool have never played on April 15 since and Dalglish, who was in his first spell as Reds manager when the disaster happened, said: "We won't play on the 15th.

"The FA and UEFA have always been exemplary and very supportive, and that's great credit to them," the Liverpool and Scotland great added.

"We've never played on the 15th before, have we? So there you are. Why would they change now?"

As for the prospect of a derby Cup semi-final, Dalglish said: "For us as the people of Merseyside it would be a game to be proud. If it is a Merseyside one then fine but it is going to be a difficult one for Everton.

"Merseyside will be hoping Everton get through to give them a day out again. But it is a difficult place to go Sunderland to get a result but that's not our problem, that's theirs to sort out for themselves."

After claiming the League Cup with victory over Cardiff City in the final at Wembley, Dalglish believes their performance against Stoke proves they are not content with just one trip to the showpiece stadium this year.

"It is the same incentive for everybody to get to Wembley and play at Wembley," he said. "We were never any less hungry for it or that performance would have showed that.

"We know we have got either Everton or Sunderland but the most important thing is that we got there.

"It is going to be a difficult game no matter who it is."

Luis Suarez opened the scoring in the 23rd minute, curling the ball into the bottom corner after a neat one-two with Maxi Rodriguez.

It took Stoke just three minutes to equaliser with former Liverpool striker Peter Crouch nodding in Matthew Etherington's corner before Stewart Downing's second-half effort, just his second goal for Liverpool, secured victory.

"I think it was a fantastic performance in many ways for us," Dalglish added. "It showed what it meant to them to get through this tie."

Stoke manager Tony Pulis praised his team's supporters, some of whom have followed the Potters on their Europa League travels, after what was their fourth away tie of this season's FA Cup.

Now he is hoping they can inspire Stoke to wins in the final few games of the Premier League season even though the Potters are 14 points above the drop zone and are now just four adrift of Pulis's 40-point safety target.

"When you're playing against one of the top teams in the country, they punish you," Pulis said as he reflected on the Cup exit of Stoke, last season's beaten finalists. "We move on and that's what we have to do.

"The supporters have been the bedrock of what this football club has done over the four or five years. We need them to be with us and get behind us for the last few home games. We still need to get the 40 points."

NEW YORK (Reuters) - Collapsing natural gas prices have yielded an unexpected boon for North Dakota's shale oil bonanza, easing a shortage of fracking crews that had tempered the biggest U.S. oil boom in a generation.

Energy companies in the Bakken shale patch have boosted activity recently thanks to an exceptionally mild winter and an influx of oil workers trained in the specialized tasks required to prepare wells for production, principally the controversial technique of hydraulic fracturing.

State data released this month showed energy companies in January fracked more wells than they drilled for the first time in five months, suggesting oil output could grow even faster than last year's 35 percent surge as a year-long shortage of workers and equipment finally begins to subside.

As output accelerates, North Dakota should overtake Alaska as the second-largest U.S. producer within months, extending an unexpected oil rush that has already upended the global crude market, clipped U.S. oil imports, and made the state's economy the fastest-growing in the union.

Six new crews trained in "well completion" -- fracking and other work that follows drilling -- have moved into North Dakota in the past two months alone, according to the state regulator and industry sources. Back in December, the state was 10 crews short of the number needed to keep up with newly drilled wells.

"Three to four months ago, the operators were begging for fracking crews," said Monte Besler, who consults companies on fracking jobs in North Dakota's Bakken shale prospect. Now "companies are calling, asking if we have a well to frack."

For the last three years, smaller oil companies with thin pockets were forced to wait for two to three months before they could book fracking crews and get oil out of their wells. As more and more wells were drilled, that backlog has grown.

Last year, an average 12 percent of all oil wells were idled in North Dakota. Even so, output in January hit 546,000 barrels per day, doubling in the last two years and pushing the state ahead of California as the country's third-largest producer.

FEWER WELLS IDLE

Fracking, which unlocks trapped oil by injecting tight shale seams with a slurry of water, sand and chemicals, has drawn fierce protests in some parts of the country, but it has not generated heated opposition in North Dakota.

The number of idle wells waiting to be completed in the state reached a record 908 last June, the result of a new drilling rush and heavy spring floods. Only 733 wells were idle in August as crews caught up, but the figure crept steadily higher until the start of this year.

Now, the industry may be turning a corner in North Dakota, the fastest-growing oil frontier in the world.

"Both rig count and hydraulic fracturing crews are limiting factors. Should they continue to rise together, production will not only increase, it will accelerate," said Lynn Helms, director of the state Industrial Commission's Oil and Gas Division.

The tame winter likely played an important role in helping reduce the number of idle wells -- those that have been drilled but not yet fracked and prepped for production. That number fell by 11 in January, as oil operations that would normally be slowed by blizzards were able to carry on, experts said.

Residents of the northern Midwest state -- accustomed to temperatures as low as minus 40 degrees Fahrenheit (-40 Celsius) in winter and snow piles as high as 107 inches -- this year enjoyed the fourth warmest since 1894, according to the National Weather Service.

The milder conditions also helped prevent the usual exodus of warm-weather workers that occurs when blizzards set in.

"Not everyone wants to work in North Dakota in the winter," Besler said.

The backlog of unfinished wells has also begun to subside because the pace with which new wells are drilled has leveled off. The state hasn't added new rigs since November.

The latest state data shows oil companies brought 37 new rigs to North Dakota's in 2011 but have not added more since November. The rig count held steady at 200 in January 2012, although more than 200 new wells were drilled in that period.

SLUMPING NATGAS PRICE PROVIDES RELIEF

North Dakota has gotten a boost from the fall-off in natural gas drilling due to the collapse in prices to 10-year lows. Energy companies such as Chesapeake and Encana have shut existing natural gas wells and cut back on new ones. Last week, the number of rigs drilling for gas in the United States sank to the lowest level in 10 years as major producers slimmed down their gas business, according to data from Houston-based oil services firm Baker Hughes. Fracking in oil patches is similar to the process used in gas wells, except for the inherent power of the pumps employed. So big service firms such as Halliburton, Baker Hughes and Schlumberger? are reshuffling crews from shale gas fields to oil prospects in the badlands. "We have moved or are moving about eight crews. Some of those crews are moving as we speak," Mark McCollum, Halliburton's chief financial officer, said at an industry summit in February.

Halliburton declined to specify where the crews were moving.

Calgary-based Calfrac moved one crew into the Bakken in late 2011, according to an SEC filing. Privately owned FTS International no longer works in the gas-rich Barnett shale but has set up operations in the Utica, an emerging prospect in Ohio and western Pennsylvania, according to a company representative.

The reallocations come with some efficiency losses. Halliburton had to scale back its 24-hour operations and is still trying to solve logistical problems. "You actually take the crew from one basin and they have to go stay in motels, you have to pay them per diems for a while. And then you have to double up your personnel while you're training new, locally based crew on the equipment once it is moved," McCollum said.

At the same time, a shortage of key equipment such as pressure pumps is easing as companies start taking delivery of material ordered months or even years ago.

It takes about 15 such pumps to frack a gas well, and many more for oil wells. The total pressure-pumping capacity in the United States at the end of 2012 will be 19 million horsepower, two-and-a-half times more than in 2009, according to Dan Pickering, analyst with Tudor Holt and Pickering in Houston.

FRACKING AROUND THE NATION

Easing personnel constraints suggest recruiters may be meeting with success in nationwide campaigns to attract workers with specialized knowledge of complex pumps and hazmat trucks -- and a willingness to brave harsh conditions.

Even with U.S. unemployment at 8.3 percent, such skilled labor remains in short supply despite salaries from $70,000 to $120,000 a year. In North Dakota, unemployment was just 3.2 percent in January, the lowest rate in the nation.

Fracking crews, much like roughnecks on drilling rigs, clock in 12-hour shifts for two straight weeks before getting a day off. They live in camps far from cities and towns. Jobs are transient -- a few weeks at a single location. Most workers divide their time between the California desert, Texas ranchlands and the freezing badlands of the Midwest state.

Companies have scrambled to nab talent, with recruiters scouring far and wide. Military bases have gotten frequent visits, and some companies have hired truckers from Europe.

"There's definitely a push to look all over for people who have good experience since it takes at least six months to train someone how to use a fracking pump," said David Vaucher, analyst with IHS Cambridge Energy Research.

TOKYO (Reuters) - Asian shares fell on Monday as investors paused to assess the effect of strong U.S. jobs data, which scaled back expectations for more easing ahead of this week's Federal Reserve meeting, while uncertainty over Chinese growth also weighed on sentiment.

The MSCI Asia Pacific ex-Japan index .MIAPJ0000PUS eased 0.6 percent after rising as much as 1.3 percent on Friday. The index has risen about 12 percent this year.

Japan's Nikkei average .N225 rose to a fresh seven-month high, after jumping more than 2 percent at one point on Friday to top the 10,000 mark for the first time in seven months. .T

The dollar hovered around three-week highs against a basket of major currencies on Monday, but slipped against the yen to 82.15 yen, after touching a near 11-month high of 82.64 yen on Friday. The euro eased 0.2 percent to $1.3094, after hitting a four-week low of $1.3085. <FRX/>

"The markets were due for a correction after rising too strongly so far, bringing prices to levels which wouldn't be sustainable," said Naohiro Niimura, a partner at research and consulting firm Market Risk Advisory Co.

Greece averted the immediate threat of an uncontrolled default when a sufficient number of private creditors agreed on a bond swap deal on Friday that will cut the country's public debt and clear the way for a new bailout.

While concerns about Europe's debt crisis will not fade away with the completion of the Greece debt swap deal, the focus will likely shift to global growth and monetary policy, Barclays Capital analysts said.

Data on Friday showed solid growth in U.S. nonfarm payrolls, suggesting the world's largest economy was strengthening and in less need of further monetary stimulus from the Fed, which holds its policy meeting on Tuesday.

While U.S. data continued to show a recovery trend, economic conditions were dire in Europe and mixed in China.

China's trade balance plunged $31.5 billion into the red in February as imports swamped exports to leave the largest deficit in at least a decade, data over the weekend showed. Recent data showed China's inflation cooled in February, while retail sales and industrial output came in below forecast.

"Aside from healthy demand for industrial metals, assessment of recent Chinese data is mixed, and more figures are needed to clarify the uncertainty," Niimura said.

In a wide-ranging statement highlighting its goals for 2012, the People's Bank of China (PBOC) promised to reduce state control over China's interest rates and currency markets to allow market forces to have a bigger play.

It also said Beijing has ample room to further cut the reserve requirement ratio for banks, adding that any decision would be based on market liquidity conditions and foreign exchange inflows.

The trade data "means bigger need to stimulate domestic demand - via fiscal stimulus and monetary easing. So we expect 200 bps more in RRR cuts and 50 bps in interest rate cuts later this year," said Dariusz Kowalczyk, senior economist and strategist for Asia ex-Japan at Credit Agricole CIB, in a note. Following Monday's PBOC comments, "We expect the PBOC to loosen liquidity in line with slower growth and inflation," he said.

Copper fell 0.5 percent to $8,454 a tonne.
